M1a – Member Data Record for Employed Members that can be downloaded from the website or can be secured from any PhilHealth office and information Service Desks in your area.
Effective December 1999, PhilHelath implemented a unified benefit package for all PhilHelath members. This package includes the following categories or personal health services:
Inpatient hospital care
1. Room and board
2. Services of health care professionals
3. Diagnostic, laboratory and other medical examination services
4. Use of surgical or medical equipment and facilities
5. Prescription of drugs and biologicals, subject to the limitations stated in Section 37 of the National Health Insurance Law or Republic Act (RA) 7875
6. Inpatient education packages
Outpatient care
1. Services of health care professionals
2. Diagnostic, laboratory and other medical examination services
3. Personal preventive services
4. Prescription of drugs and biologicals, subject to the limitations stated in Section 37 of the National Health Insurance Law or Republic Act (RA) 7875
5. Emergency and transfer services
